Names and contact details of next of kin must also be provided

The Meritorious Service Medal (MSM) is a military award presented to members of the United States Armed Forces who have distinguished themselves by meritorious service or service in the United States after January 16, 1969.
The MSM was previously awarded as a decoration for peacetime achievements, but effective September 11, 2001, the decoration may be awarded in lieu of the Bronze Star medal for meritorious achievement in a specific combat theater.
